1. Parties and authority

I, ______________________________ ("Parent/Guardian"), of ______________________________ (address), am the ☐ parent ☐ legal guardian of ______________________________ ("Player"), Division: ______________, and I have full legal authority to sign this Release on the Player's behalf.

2. What I am deciding

Chandler National Little League ("the League") photographs, records video, and (on League-controlled channels) streams games and events to celebrate its players and promote League activities. Under federal law (COPPA) and Little League policy, the League needs my written permission before publishing identifiable images, video, or audio of my child, or my child's name. Each permission below is my separate choice. My child's opportunity to play is exactly the same whichever boxes I check: including none.

3. My permissions (check each choice)

A. Photos, video & audio: The League may photograph and record the Player during League games and events and use those materials to promote League activities, at the League's reasonable discretion, in League publications, League media releases and announcements, the League website, and League social media pages. ☐ YES, I give this permission ☐ NO, do not publish identifiable images of my child (the League will honor this on the website, on League social media, and in League publications, and team volunteers will be notified)

B. Name on rosters & results: The League may list the Player's name (first name and last initial, or full name where customary, e.g., printed programs) on the team rosters and event results it publishes. The League's website (www.chandlernational.com) publishes no rosters, team pages or results today; this permission covers the rosters and results the League publishes elsewhere, and it covers the website if that ever changes. ☐ YES ☐ NO, do not publish my child's name

4. Scope of the grant

For each permission I checked YES, I grant the League only a non-exclusive, royalty-free license, for the duration stated in Section 6, to use, reproduce, and display the described materials, including the Player's image, likeness, name, and voice as applicable, solely to promote and operate League activities. I agree such League-created materials are the property of the League.

Express limits, which the League has adopted as policy and reaffirms here:

• This grant extends to no other entity: not sponsors, not commercial entities, not other organizations. Any third-party or commercial use would require a separate, specific written consent from me and any approval required by Little League International.
• The League will never sell, rent, or share the Player's images, video, or information with any third party for fundraising, marketing, commercialization, or promotional activities.
• The League will never publish the Player's contact information, home address, school, or whereabouts, and will not caption images with information beyond name (if permitted in B) and team/division.
• No use will be derogatory or contrary to the Little League Child Protection Program.
• Little League International's guidelines on the appearance of Little Leaguers in media apply to any non-news promotional use.

5. No compensation

I agree that neither I nor the Player will receive any compensation for uses permitted by this Release.

6. Duration, revocation, and my ongoing rights

• This Release covers the season identified above and remains in effect for materials created during that season until I revoke it.
• I may revoke any permission at any time, effective going forward, by written notice to contact@chandlernational.com (or the League's P.O. Box). The League will confirm receipt, stop new uses immediately, and remove affected materials from League-controlled digital properties (website, League social media) within 10 business days. I understand revocation cannot recall materials already lawfully distributed in physical form or shared beyond League control.
• My COPPA parental rights (review, correction, deletion, refusal of further use) are described in the League's Children's Privacy Notice and apply fully to materials under this Release.
• The League retains this signed form as its record of verifiable parental consent, per its written retention policy.

7. Governing law

This Release is governed by Arizona law; venue for any dispute lies in Maricopa County, Arizona.
